New Patient Forms

To schedule an appointment:
1. Call our office at 610-828-7595. At that time we will verify the coverage of your health insurance plan or arrange out of pocket payment and schedule you for an initial
evaluation. Conshohocken Physical Therapy is a Direct Access licensed facility so you do not need a Physician's prescription until after your first 30 days of treatment.

2. Print the New Patient Forms below and bring them to your first visit. This will allow us to better serve you.

Health Insurance/Payment Options
Below is a list of the most commonly accepted health insurance companies. Additional health insurance plans are accepted. When you call to schedule your initial
evaluation we will verify the coverage of your health insurance plan or arrange out of pocket payment.

Some accepted insurance plans:
• Aetna PPO, Open Choice, Managed Choice • Amerihealth • Blue Cross/Blue Shield • Cigna • Devon • Healthnet • Medicare • Motor Vehicle • Personal Choice
Personal Choice 65 • Private Healthcare Systems (PCHS) • Tricare • Workers Compensation. Additional Insurances are accepted.

Self Pay:
If you do not have or choose not to use your health insurance our out of pocket rate is $125 for the initial evaluation (1st visit), then $95 per follow up session.

Patients choose Conshohocken Physical Therapy because they receive the time and attention they need recover. We will teach you how to prevent and manage a
condition so that you will achieve long-term health benefits. We are a Direct access facility and can treat you without a referral for the first 30 days.
