| To schedule an appointment: 1. Call our office at 610-828-7595. At that time we will verify the coverage of your health insurance plan or arrange out of pocket payment. Conshohocken Physical Therapy is a Direct Access licensed facility so you do not need a doctors prescription until after your first 30 days of treatment. 2. Print the New Patient Forms and bring them to your first visit. This will allow us to better serve you. 3.
